Booking & Service Information
Your quote identifies the services, schedule, pricing, and agreement version that apply to your project. Please review the complete documents provided with your quote before accepting.
Requesting a quote. Tell us your service location, estimated tests or program-service needs, and preferred timeframe. Submitting an inquiry does not reserve an appointment or commit you to a purchase. Availability and the final scope are confirmed in your proposal.
Confirming the scope. Review the included testing, service window, travel, medical evaluations, records delivery, and any additional work. For reserved coverage, confirm the period we will be available and what is included during that time.
Preparing for service. Coordinate participant readiness, required medical authorization, respirators, the testing space, and an on-site contact. Raise scheduling or preparation questions before your session.
Project terms. Payment, cancellation, rescheduling, additional time, and other project-specific terms are stated in your accepted quote and applicable service agreement. This page is general booking information; it does not replace or amend those documents.
